The ARC Hub PCN Lifestyle Medicine Programme
Transforming healthcare through community-rooted, evidence-based lifestyle interventions.
Our Vision
To establish ARC Hub PCN as a national leader in the delivery and research of lifestyle medicine.
We aim to integrate behaviour change science, physical activity, nutrition, group consultations, and social prescribing to improve outcomes for people living with long-term conditions, while reducing health inequalities across Northamptonshire.
Our Mission
To empower individuals and communities through evidence-based, multidisciplinary care that supports physical, emotional, and psychological wellbeing.
By combining NHS services with local community assets and academic partnerships, we aim to build a scalable, replicable model of integrated care.
Our Core Values
Equity – Reaching marginalised groups with inclusive, culturally sensitive interventions
Empowerment – Supporting patients as active participants through education, coaching, and peer support
Collaboration – Partnering across NHS, academia, charities, and local authorities
Sustainability – Scalable, realistic models aligned with NHS goals and Greener Practice
Innovation – Embracing digital tools, group models, and research integration
Evidence-Led – Informed by NICE guidance, behavioural science, and real-world data
Strategic Pillars
These 6 key pillars mark the start of our lifestyle medicine journey, and set out our key focuses over the next year:
Integrated GLP-1 Clinics – Group onboarding & wraparound care for patients on tirzepatide (Mounjaro)
Sleep & Mental Health – CBT-i, deprescribing, mindfulness & physical activity integration
Menopause & Women’s Health – Holistic care combining HRT with nutrition & strength training
Pain & MSK – Functional movement & pain education via group consultations
Lifestyle Medicine Training – Upskilling clinicians in coaching, MI, and behaviour change
Research & Innovation Lab – Partnering with NIHR to evaluate and share impact nationally
Our Philosophy
We believe lifestyle change is not optional, it is essential.
Our approach is rooted in:
Neuroscience and behavioural economics
Co-production and patient voice
Long-term sustainability
Community-rooted delivery
We act now to reshape how general practice responds to chronic illness, giving patients agency and restoring joy and purpose to primary care.
Scaling & Sustainability
We are building a realistic, sustainable model of care, fit for today’s NHS.
Starting with focused pilots, we will:
Work with NHS England, ICB, and local partners to scale across Northamptonshire
Share learning with PCNs across the region
Build an outcomes-led business case with academic partners
Seek long-term support from bodies such as NIHR, Sport England, and legacy funds
Our ambition: a dynamic community of practice that champions lifestyle medicine across primary care.
